Completing and Submitting a Web Form

Complete the short exercises below.

Radio Buttons
For the question below, indicate that you have never completed a web form, by selecting "No".  Click in the radio button in front of the word "No" to select the option.

Data Fields
(Also called textboxes or data entry fields)
To move to the data fields, move your cursor to a box below and click.  A cursor will appear in the box, enabling you to use your keyboard to enter the data. Try typing in your name below.

Pull-Down Menus
In the pull-down menu below, click on the arrow to the right of the input box to see the available options.  Click on the option of your choice to select a response. Try it below.

Checkboxes
When a checkbox option is selected, a checkmark will appear in the box that corresponds to the desired choice.  To select an option, click in one of the boxes below.  In some cases, you may be able to select more than one option.  In the example below, select "ARES, "FRS", and  "ELF".  Thereafter, deselect one or more of the options, by re-clicking in the checkbox.

Submitting the Form
At the end of the form, you will normally have two options, one to submit the form and one to clear the form (allowing you to redo the form). The following are typical options that appear at the bottom of web forms.
